Upon admission the social worker will conduct an initial assessment to obtain collateral information about the patient’s social environment from both the patient as well as the family.
This information may be obtained from the family or next of kin either during a in person meeting or a telephonic discussion.
The social worker will be rendering supportive services to both the patient as well as the family throughout the rehabilitation process. This includes counselling, therapy, family feedback as well as assisting with finding placement in a facility should the patient need it.
